As A Job Performer my Responsibility is..."
JP :
"Job Performer needs to commence the work in a safe condition related to the worksite with tools and equipment, conduct tool box talk with work groups and keep on monitoring the site activities."
- "JP accepts the permit from PI (Permit Issuer) after checks the worksite is clear and maintained in safe condition with PI."
- When the permit needs to be revalidated (Next shift), JP needs to inform PI (Permit Issuer) & AA (Area Authority) to get the permit revalidated (For 12 hours).
- When the permit needs to be closed (Permit is expired after 7 calendar days), JP needs to suspend the work and inform PA (Performing Authority) that the work is done and the worksite is clear and maintained in safe condition. Then PA requests for the closure of the permit to PI & AA.

"Performing Authority needs to provide a clear and adequate description of the work to be carried out with sufficient details, and ensure that permit request is complete with up-to-date JSA covering full work scope ALARP attached to the permit."
2. What is WMS?
"Work Management System : It is a safety tool that the work is properly Controlled, Coordinated and Communicated (3C)."
3. What are the modules of WMS? (5 modules – name all of those)
- PTW (Permit to work),

- SIMOPS (Simultaneous Operations)
4. What are the Life saving rules (At least 5 of them)?

7. What is the whole process of PTW (From beginning to End of PTW process)
Ex) PA requests for the permit.... (Better explain regarding the 8 steps of PTW)
- General Work Permit (JSA-1) :
Request (PA) – Validated (AA) – Endorsement (PE) - Authorized (AA) – Permit Issue (PI) – Permit accept (JP) – Permit suspend (JP) – Request for PTW closure (PA) – PTW Closure approved (PI & AA)
- Critical & Hot work permit (JSA-2 & FRA) :

8. What is IWAP? & When do we need IWAP?
- After PA requests (drafts) the permit, IWAP (Integrated Work Activity Planning) needs to be conducted (Only one time) to decide which type of permits / JSA levels should be implemented into the specific work that is being addressed.
- Required documents : WCC (Work Categorization Chart), MES (Method Statement), Certificates (If required)
9. For how long the validated PTW can be extended?
"A validated PTW (Permit) is valid for 7 calendar days. One shift is 12 hours, and it can be extended for additional 4 hours (16 hours in total)"
10. How many different JSAs are there? & What are the differences among JSA-1, JSA-2 and FRA?
- JSA Level 1 : General Work Permit / The job activity does not generate heat, flame and spark and has low risk in nature and there is no more critical condition such as toxic gases and flammable hydrocarbons.
- JSA Level 2 & FRA : Hot Work Permit / The work activity generates a heat (Welding, Cutting and Grinding) Critical Work Permit / The work activity consists of high risk and critical situations such (Spark potential, Breaking containment, Working underwater, Working on energized power line and Critical lifting etc.)
11. What is SIMOPS? And who is going to be SIMOPS coordinator? What is MOPO matrix?
- Simultaneous Operations : 2 or more than 2 different works that can interfere one another are going on at the same time in the same place.
- SIMOPS Coordinator is AA (Area Authority).
- MOPO matrix (Matrix of Permitted Operations). By looking at the specified colour in the matrix, we can figure out whether the SIMOPS work can be commenced or not.
Green : Good to go / Yellow : Restricted / Red : Prohibited
12. What are the two types of SIMOPS?
- Routine work : Activities able to be pre-identified and included MOPO (Manual of Permitted Operations)
- Non-Routine work : Activities not yet covered by existing formal risk assessment (JSA/FRA)
13. What is Dynamic risk assessment?
"Identifying hazards and assessing risks from changed circumstances. It shall be required to be done all the time whenever the circumstances are changed."
14. What is TD (Temporary Defeat) and the process? What are the equipment going to be applied with TD? – HSECES (HSE Critical Equipment & System), What are they?
- Bypassing the HSECES when any works should be done at where the equipment or facility being protected remains in service.
- AA (Area Authority) -> DA (Defeat Authority) -> DP (Defeat Performer) -> JP
- *HSECES are installations such as plant equipment, structures and systems (Including computer programs) that either its failure can cause Major Accident and its purpose is to prevent/mitigate consequence of major accident.
- Examples: Pressure safety valves, Emergency blow down systems, Dykes, Pits, Deluge, Foam systems, etc.)
15. What is EI (Energy Isolation) and the process?
- Isolating the energy from the source to execute the work safely.
- AA (Area Authority) -> ISA (Isolating Authority) -> ISO (Isolating Operator) -> JP
16. What are the precautions for critical & hot work?
"PPE, Toolbox talk, Safety devices, Energy isolation, Safety signage and Work procedures"
17. When do we not need permits?
- Routine Activities,
- Low Risk involved,
- Single Discipline,
- Completed in less than a shift,
- No impact on operations
