What is Permit To Work (PTW)
A set of documents to maintain safe working conditions for a particular task. A PTW will:
- Describe the work
- Ensure correct authorization for the work is in place
- Ensure hazards are identified and controls implemented
- Be a written agreement between involved parties
- Become a saved written record of work performed
- In the event the electronic PTW system is unavailable, a paper based PTWsystem, the PTW Crash Pack, shall be provided as a contingency
- All permits and certificates shall be hand-written on the pre-printed formsof the Crash Pack
- Authorization is required from the Asset Owner with endorsement fromSite HSE Manager in order to use the Crash Pack

"A competent person appointed in writing by the Asset Owner and responsible for monitoring the safety of an area and the assets within it and providing clearance to work in that area at any particular time".
Note: For non-operational areas, the PI is appointed by the Asset Owner for that particular area (Maintenance, Engineering or Construction or Electrical).

Job Performer (JP)
Maintenance Technician
A person who works under the direction of Performing Authority (PA) and accepts the worksite as safe to commence work. He communicates the hazards, permit controls and conditions to other work party members or group or personnel.

PE are personnel who are affected/ impacted by activities described in work permits (other than IA, JP, AAA and PI of the same permit) or personnel whose expertise may be necessary for defining / approving controls. PE review of the work scope and the controls identified to mitigate interface / concurrent activities and agree that the work described may be carried out during the proposed period.
Radiation Protection
Officer
A competent person responsible for reviewing the information provided in the radiation certificate, supporting documents and work site precautions and confirming that the work site preparations and plans are adequate and equipment to be used for the work is suitable for use on the worksite.
Authorized Fire Personnel
A competent person responsible for identifying required work site precautions and control measures for safe execution of work related to road closure.
Authorized Gas Tester
"A competent person responsible for assessing and identifying requirements for the safe execution of work, verifying the acceptable conditions, ensuring test requirements are fit for use (calibrated and functional) and communicate abnormal atmospheric test conditions".
Authorized Civil Personnel
A competent person responsible for risk identification, assessment and controls specific to trenching collapse, planning and preparing for safe trenching collapse, identifying underground services, soil types, and other factors related to stability of trench.
Confined Space Entry
Observer
A competent person responsible for keeping track on the personnel involved during work in confined space, maintaining communications and visual contact with all personnel including the emergency response team and main control room, ensuring all equipment are readily available (PPE, RPE, BA, safety harness, lifting equipment) and recording & reporting any incidents during work.

A competent person within the PTW System (employee or contractor as applicable), assigned to request a permit or supervise Job Performers.
▪ For permit request:
- Provide a clear and adequate description of the work to be carried outwith sufficient details.
- Ensure PTW request is complete with up-to-date JSA covering full work scope.
- Declares competence of the Job Performer identified for the work activity.
▪ During execution of work:
- Adhere to PA worksite presence requirements as specified by AA on the permit forms.
- Sign on TBT form to record worksite presence.
▪ For permit closure:
- Confirm with JP about worksite conditions, work and equipment status.
- Request closure of permit.
- Provide reason if the work is not completed
Asset Approval Authority (AAA)
A competent person approved in writing by Asset Owner who shall sign all Critical and Hot Work Permits confirming that the described controls are adequate to reduce the risks to ALARP. AAA shall:
- Ensure work scope has been clearly and correctly defined and correctwork categories assigned.
- Review JSA to ensure potential hazards, precautions, conditions, and controls are defined meeting ALARP.
- Identify whether the planned worksite preparations are adequate.
- Assess whether the proposed supervision levels are adequate.
- Check SIMOPS management.
- Management of deviations outside of this standard.
- Approve Temp. Defeats as required by defeat approval matrix.
Note: As a minimum, the AAA shall be one level above the Area Authority
Area Authority (AA)
AA is responsible for the authorizing a permit having confirmed that the pre-requisite controls are in place and proved effective. AA is also responsible for managing the re-validation, suspension, and closure. AA shall:
▪ Prior to Permit Authorization:
- Check permit information and JSA is appropriate for the described scope of work.
- Ensure all required documentation (supporting certificates, attachments, etc.) is attached to the permit and fully endorsed.
- For Critical or Hot Work, verify JSA content and effective implementation of the pre-requisite controls.
- For General Work, review JSA content and provide approval for ALARP.
- Manage Simultaneous Operations (SIMOPS) and ensure communication protocols are defined.
- Review, verify and approve the scope of the isolations /de-isolation plan and ensuring correct sequencing of isolations.
- Identify if independent verification is required for the isolation.
- Confirm all isolations are in place and proven effective and sign off the Isolation Certificate (IC)
- Approve Temp. Defeats as required by defeat approval matrix.
- Specify the time duration of permit validity.
- Provide instruction that work shall not commence until formal clearance has been provided by PI.
- Inform parties affected by the work activity (i.e., Area Authority, other Interface Authorities, PE, etc.).
- Revalidate, suspend, and close permits as required.
- Confirm all PTWs dependent on an IC are closed prior to decisions to de-isolate.
- Authorize Sanction to Test (STT).
- Authorize de-isolation.
- Transfer IC to/from Extended Period Isolation.
- Close the IC.
A Job Performer (JP) is responsible for the acceptance of a permit and supervising the safe conduct of the work activities by implementation of all relevant permit defined worksite controls including informing all members of the work party. JP shall:
▪ Prior to commencement of work:
- At the worksite immediately before work is to commence, perform (together with PI) a site situational hazard check, work party and equipment checks.
- Abide by the directions of the PI regarding clearance to commence work or instruction to suspend or stop work.
- Declare that all work party members cleared from the site and personal locks removed, work area left in a clean state and equipment left in a safe condition.
PI is responsible for the field operation or specific work location of the asset including the safety of all persons working within that asset area. PI shall:
- Undertake situational site safety hazard checks.
- Demonstrate zero energy to JP when applicable.
- Confirm that the local site preparations specified on the permit are visibly in place. ▪ Discuss with the JP the nature of the work being undertaken in the current shift period. ▪ Confirm that the work teams are fully informed on the necessary precautions for the work area including emergency response measures and safe evacuation protocols.
- Prior to commencement or recommencement of work ensuring all atmospheric tests are undertaken and levels recorded and safe for work to proceed.
- Provide a clearance for work to commence.
- Maintain oversight of the safe conduct of work activities during their shift.
- Ensure that all information related to status of work activities in their area of responsibility is fully recorded in shift logs / handover documentation.
